Tibial Nerve Block- Medial Prone Approach

Scott Thomas, former regional anesthesia fellow at the Andrews Institute, demonstrates an ultrasound guided foot-drop-sparing tibial nerve block as an alternative to sciatic block for secondary pain control for ACL reconstruction. Dr. Thomas uses a prone approach which allows him to remain on the same side of the patient for the placement of a femoral catheter. He also uses a low volume medial to lateral needle approach, maintaining local anesthetic on the medial side of the tibial nerve in hopes of less local anesthetic spreading up the sheath to block the peroneal nerve laterally.
